You will need the following information to book your appointment and at the time of visit:
Insurance Card, if you have a secondary insurance, please provide that as well
Social Security number
Valid email address to receive your new client paperwork via our portal
Driver’s license information
Once your appointment is booked please be on the look out for your new client paperwork via email. You will login to your portal to complete this. If the paperwork is not filled out prior to your appointment you will need to arrive at least 15 minutes prior to your appointment to get this done before being seen by our providers.
